The Saturn 1B package. It was mostly used for unmanned tests of the command and service modules, but it did send 5 manned flights into orbit: Apollo 7, the joint Apollo/Soyuz mission and the 3 Skylab missions. There was never a launch failure of either the Saturn 1B or the Saturn V
